However, the foundation for the development of some rural households is still relatively fragile, and some rural households may have serious difficulties in their basic livelihood due to causes, disasters, accidents, etc., and if they cannot receive timely assistance, they are prone to return to poverty and become poor. In accordance with the requirements of the CPC Central Committee, during the transition period, we have established and improved a dynamic monitoring and assistance mechanism to prevent the return to poverty, and the relevant policy points are hereby notified as follows.
1. Application requirements
The monitoring scope of poverty prevention (the annual per capita net income of Wudu District in 2024 is 7,800 yuan) is a reference for judging whether it can be the subject of poverty prevention monitoring. In principle, the subject of the application for monitoring should have a rural household registration and have one of the following risks of returning to poverty and causing poverty.
1. There is a security problem in the family's only house, and the family is unable to solve it;
2. The family is unable to solve the difficulties encountered in the draft of the family, including water interruption for more than 1 month, water quality problems, long distance (long time) of water intake, etc.;
3. Family members suffering from serious illnesses, serious illnesses, long-term chronic diseases, etc., and the family cannot afford the high medical expenses;
4. The cost of school for students at home is large, and the family cannot afford it;
5. The family's income has been greatly reduced or expenses have increased significantly due to disasters and accidents, which has affected the basic life.
2. How to apply
You can apply in person or by a representative in the following ways:

3. One-code pass. Scan the QR code of "Gansu One-click Poverty Report" on your mobile phone to apply.
III. Instructions for Application
1. If you apply for monitoring subjects, you need to cooperate with us to carry out household verification, you need to truthfully and accurately provide information about the family and members, and authorize us to verify and verify the income, property and other relevant information of the family members in accordance with laws and regulations, and after the village reviews and publicizes, the district and township will review and approve it level by level.
2. If it has become the target of monitoring, we will select corresponding measures to help from the aspects of housing security, drinking water safety, health assistance, education assistance, industrial assistance, employment assistance, financial assistance, comprehensive security, and social assistance according to the actual difficulties and outstanding problems.
3. If the problem of family difficulties has been resolved, and the annual per capita net income has stabilized beyond the scope of monitoring for the prevention of returning to poverty in that year, after household verification, evaluation and publicity, and review and approval, it will be determined that the risk of returning to poverty has been eliminated, and no targeted assistance will be provided thereafter.
4. After the risk is eliminated, if there is a new difficult problem in the family, the application can be made again.

"Gansu one-click poverty report"
The "Gansu One-click Poverty Report" system is a system platform for rural households to declare the monitoring objects to prevent them from returning to poverty, which has the advantages of simple operation and strong effectiveness.
If the per capita income of family members living together in agricultural household registration is lower than the local monitoring standard for preventing poverty return (7,800 yuan in 2024), and if there are serious difficulties in basic living due to large rigid expenditures such as causes, disasters, accidents, etc., or a significant reduction in income, they can self-declare the monitoring targets through the "Gansu One-click Poverty Report".
The specific application process is as follows:
The first is to scan the code to declare. The family members of the applicant farmers use their mobile phones, WeChat or mobile browsers to scan the QR code for self-declaration. For special persons who are unable to apply due to disability, serious illness, etc., others can apply on their behalf.
The second is to fill in the information. After scanning the QR code, fill in the applicant's 4 pieces of information (the name of the head of household, ID number, mobile phone number, and home address).
The third is to fill in the information and submit the application. Check the entered application information, confirm it, and submit it for processing.
After submitting the declaration, the applicant must authorize in writing to agree to the verification of the family financial status of the applicant and all family members. Within 5 working days of the farmer's application, the village-level staff will carry out a family status check. After the verification is completed, the staff will give feedback on the verification results to the applicants, and if they do not meet the criteria for the identification of the monitoring subjects for preventing the return to poverty, they will be informed of the reasons for not meeting the acceptance. If the applicant has any objection to the verification result, he or she may directly submit an application for review to the local township.
