01. PPE

1. The helmet has no aging, no damage, and no human maintenance and transformation.

2. Wear the helmet correctly: Wear the hat properly and fasten the hat strap tightly.

3. Wear qualified safety glasses.

4. Wear the face shield properly. Such as grinding, cutting, crushing and other operations.

5. Safety glasses should also be worn under the welding mask. Soldering assistants also use sunglasses.

6. Choose and wear appropriate protective gloves according to different types of work, and each shift must ensure that your employees have gloves.

7. All personnel on site should wear reflective vests.

8. Do not roll up the sleeves of your work clothes and do not leave your clothes open to prevent direct skin exposure to hazards.

9. Drivers and others should use safety belts, wear PPE and close the doors when getting out of the vehicle.

02. Work at height

1. The seat belt is unqualified, damaged, and there is no inspection mark, if the team has an unqualified seat belt or no label, it must be replaced in time.

2. Working more than 1.8 meters on mobile scaffolding without proper use of safety belts, safety belts should be hung at a high place.

3. When stacking materials, when loading and unloading materials, no fall protection is used when exceeding 1.8 meters

4. There is no warning area under the work at height, or the safety range of the warning area is insufficient (2 times), and the warning area is not closed and there is a gap

5. When working at height, the safety belt is not hung high and low, and the falling safety distance of the buffer belt is not taken into account.

6. The suspension point of the safety belt is unreliable and not firm, such as hanging under the steel structure, the edge of the scaffolding pipe, etc.

7. Tools and equipment do not have fall protection measures at height, such as the use of tool bags or lashing.

8. Cross-work, and the people below are not protected.

9. If the person does not achieve 100% fall protection when moving at a height, double hooks should be used alternately.

10. Materials working at height are not cleaned up in time and there is no fall protection, such as using buckets or bags to store loose materials.

11. The safe passage to and from the working floor at height was not considered before operation.

12. All passages, fire protection, and electrical box doors are not allowed to be blocked, and wires, gas cylinders or other objects are forbidden to be placed at the passages.

03. Hot work

1. The welding machine is not inspected before operation, and the welding wire joints (terminals) are in poor contact, resulting in local high temperature fires.

2. Before the hot operation, there is no inspection and cleaning within the safety distance of the surrounding area, and combustible and flammable materials are left.

3. The fire operation is not equipped with a qualified fire extinguisher, and the fire extinguisher is placed in an improper position.

4. There must be a fire guardian, and the danger can be eliminated in time during the operation.

5. The cylinder is not fixed and the fixed position is incorrect, so it should be fixed at 2/3 above the cylinder.

6. Ineligible cylinders do not have a protective steel cap. The cylinders stored in the cylinder cage are not secured.

7. There are no safety signs and warning signs at the storage of gas cylinders, and the storage distance between acetylene and oxygen is not enough.

8. During gas welding operation, the distance between acetylene cylinder and oxygen cylinder is less than 5 meters, and the distance between the gas cylinder and the ignition point is less than 10 meters.

9. The outlet hose of the gas cylinder is seriously aged, and the oxygen cylinder does not use an anti-flashback device and does not use a special hose fixture.

10. No brazier is used when working at height, or no restricted area is set up below.

11. Use power tools or hot work when flammable objects appear in the surrounding environment.

12. Smoking near the storage point of flammable and combustible materials without necessary warning signs and non-smoking signs.

04. Scaffolding

1. The scaffolding is not labeled, and the back of the label is not checked for the date.

2. The springboard is not full, can not meet the construction needs, and the lack of springboard causes holes.

3. The springboard binding wire is too thin, the springboard binding method is incorrect, and each springboard is tied separately.

4. The setting position of the upper and lower ladders is incorrect, which conflicts with the construction workpiece.

5. The ladder should only be installed at the platform, and should be more than two steps beyond the platform.

6. The baseboards are not installed correctly, and the baseboards are not installed on all four sides of the platform.

7. The springboard head is cracked and not fixed with wire binding.

8. The ground erected by the mobile scaffolding is uneven.

9. Wooden ladders were found to be used in the field.

05. Hoisting

1. The lifting object is lifted in the air, and the crane operator leaves the crane.

2. Crane operators are prohibited from leaving the cab without personal PPE.

3. There is no warning in the operating radius, and safety warning signs are set up. Workers walk around the hoisting area.

4. The lifting commander has no flag, no whistle, and no sign language.

5. Do not use a tail rope for lifting objects or use a tail rope that is too short.

6. Incorrect use of sling: lifting angular objects without using protective measures. The sling is not attached using a snap.

7. The staff passes under the hanging object, and the lifting object is processed under the hanging object.

06. Electrical

1. Power tools are not regularly inspected and equipment inspection labels are posted.

2. Non-electricians wiring and repairing power tools without permission.

3. Failure to regularly inspect and find and replace failed switching appliances in a timely manner.

4. The power cord of the power tool is damaged and not wrapped and insulated in time.

5. The installation of the switch box bracket is not stable, the on-site use is not firmly placed, sometimes dumped on the ground for use, the box door is not closed during use, the fixed distribution box is not filled in the daily inspection record sheet and the detailed contact sign and check color code are posted.

6. The cable is erected in a messy manner, no insulated S hook or triangle bracket is used, the line passing through the road and passage is not protected, or the erection height is too low not less than 4.5 meters or with eye-catching safety warning tape.

7. The electrical socket is damaged and not replaced in time, and the circuit breaker or socket in the switch box is not firmly fixed.

8. The protective housing of the lighting device is broken and the glass cover is missing.

9. The installation position of the distribution box is not suitable, and there is a lack of good operation space.

07. Excavation operations

1. The safe distance between the excavator and the edge. (1.5-2m)

2. Failure to appoint flag bearers as required.

3. The roadblock was not set up in time at the edge of the excavated foundation pit.

4. There is no suitable safety passage for personnel to enter the foundation pit and there is no collapse measure for the foundation pit.

5. When the staff is working in front of the loader, the loader does not turn off the generator.

08. Civilized construction

1. Failure to clean up the work site in time after the operation is completed.

2. The materials on the construction site are placed chaotically, the materials are not classified and stored, and they are stacked neatly.

3. The on-site passage is crowded with construction materials.

4. The on-site line/trachea is not hung up at a height of 2 meters or measures are taken to prevent people from tripping.

5. There are no garbage bins on site, or the garbage in the garbage bins is not cleaned up in time.

6. Scaffolding steel pipes and wooden planks were not stacked in the designated place in time.

7. Drip trays or anti-leakage trays are not used in areas where water or other liquids may leak/spill, and chemical liquids that leak/spill are not treated in time.

8. Gas cylinders are discarded at random and are not stored in the designated place according to regulations.

09. Tools and equipment

1. Electrician's tools are not regularly inspected and posted with color identification labels.

2. The power cord of the power tool is damaged and not wrapped and insulated in time.

3. Use homemade tools to do your homework.

4. The welding machine is not grounded when it is used, and the terminal is exposed.

5. Compressor belt/disc holder/grinder, grinder and other rotating parts of the protective cover are missing.

6. Aerial work tools are placed randomly without using tool bags, and the tools are easy to fall and injure people.

7. There is no anti-slip treatment at the foot of the A-ladder, or the top floor of the A-ladder and the following 2 floors are used for operation. When working, there is no personnel escalator ladder; Work with your back to the ladder.

8. Move the scaffolding while the person is standing on the scaffold.

10. Material operation and use and civilized construction

1. Gas cylinders should be classified and stored in the cylinder cage, and identified by name and posted on the cylinder cage together with MSDS; Oxygen and acetylene cylinder cages must be marked with a "No Smoking" sign and equipped with a fire extinguisher.

2. Empty bottles and full bottles should be distinguished, hang empty bottles and full bottles identification plates, and use two iron chains or brown ropes to separate and tie empty bottles and full bottles.

3. Gas cylinders are not equipped with a pressure reducing valve.

4. The trolley should be used for the transportation of gas cylinders, and the road conditions should not be suitable for the trolley to be carried by two people, and no single person should be resisted.

5. The cylinder must be placed vertically, and the cylinder should be tied firmly with the fixed point with an iron chain or brown rope, and the middle and upper part of the cylinder should be tied, and the bottle nozzle should not be tied.

6. Keep the distance between oxygen cylinders and acetylene cylinders 6 meters and 10 meters away from the fire source.

7. Paint must be stored in a designated chemical warehouse, with a name logo, fire prevention logo, MSDS, and fire extinguisher posted at the door of the warehouse.

8. The paint used daily on site must be stored away from the fire point, and be surrounded by warning tapes, and the name logo, fire prevention logo, MSDS, and fire extinguisher must be posted, and the designated chemical warehouse must be taken back at the end of work.

9. The empty barrels of paint barrels and other chemical containers on site must be covered with barrel lids and bottle caps in time, and the chemical warehouse must be collected every day, and empty barrels and bottles of discarded chemicals are prohibited on site.

10. When the paint bucket is used for other purposes, such as holding fasteners, bolts, water or collecting garbage nails, and stacking them neatly; and transported away from the scene in time.

11. Chemicals must be stored in the designated chemical warehouse, and the name logo and MSDS are posted at the door of the warehouse.

12. For chemicals used daily on site, the on-site storage point shall be surrounded by warning tapes, and the name logo and MSDS shall be posted, and the designated chemical warehouse must be taken back at the end of work.

13. Chemical containers must be properly lided, even for empty drums.

14. For the transportation and storage of chemicals, it is forbidden to place acids and alkalis together in accordance with the requirements of MSDS; When transporting by vehicle, the chemicals should be placed smoothly and not stacked, and the container mouth should be checked to see if it is sealed.

15. Chemicals should be stored in secondary protection containers to prevent dripping, and it is forbidden to place chemical containers directly on the ground.

16. Each team must stack materials neatly and clean up garbage in a timely manner.

11. Confined Space

1. All confined spaces must have a "Confined Space, Unauthorized Entry Prohibited" sign, and the entrance manhole must be closed and sealed.

2. Work permits, gas test record forms, and personnel entry and exit registration forms should be hung at the entrance to the confined space.

3. Confined spaces need to be prepared for ventilation, gas test qualified, 12V lighting.

4. Entrants and guardians must be trained to enter confined spaces and have a tag attached to the helmet.

5. Rescue facilities must be set up at the entrance of the confined space, such as setting up a tripod, hanging pulleys, and pulling the rescue rope, one end of the rescue rope is fixed outside the confined space, and the other end is firmly tied with the safety belt of the entering personnel.

6. Incoming wires must be isolated from metal insulation and hung neatly while preventing people from tripping.

7. Guardians must wear reflective vests, and it is forbidden to work in confined spaces without guardian supervision.
