Team Note: Jazz center Azubük triggered a health-safety protocol

According to Jazz reporter Ben Anderson, team center Azubuk triggered a health and safety agreement.

Azubuk has only played in six games for the Jazz after suffering a right ankle dislocation, averaging 1.5 points, 1.3 rebounds and 0.2 steals per game.
