On August 19, a flash explosion accident occurred in the initiator workshop of Ningxia Shunbangda New Materials Co., Ltd., resulting in 5 deaths. The preliminary analysis of the cause of the accident is as follows: in the process of configuring the lye, the valve at the bottom of the preparation kettle and the valve at the top of the storage tank are not closed tightly, which leads to the lye stringing into the storage tank, and the organic peroxide in the storage tank reacts violently to release the heat and cause a flash explosion, which causes the adjacent finished tank (70% tert-butyl hydrogen peroxide) to flash again.
