Recently, the Beijing Municipal Health Commission issued the document "Notice on Further Strengthening the Standardized Construction and Management of Village-level Medical and Health Institutions in Beijing" (hereinafter referred to as the "Notice"), which clarifies issues such as the salary and treatment of village doctors and social security.
In March this year, the Beijing Municipal Health Commission issued the "2021 Beijing Municipal Grassroots Health Work Points", which highlighted the strengthening of rural health services, and set up village-level medical and health institutions in accordance with the principle of "one village, one room (station)" for 578 administrative villages without village-level medical and health institutions.
Now that the policy has finally landed and it is clear that it will be fully implemented by November 30, what changes will be ushered in by village-level medical institutions? And what are the specific measures to benefit village doctors? Let's take a look.
"Town management village use", open recruitment
In order to ensure that village-level medical and health institutions, especially village clinics, are responsible for and on duty, and to ensure that public health and basic medical services are in place, the Notice mentions that the implementation of the "integrated management of people, finances and materials" of medical and health institutions at the town and village levels in the form of village clinics dispatched by township health centers is gradually realized.
According to the principle of "town management and village use", each administrative village is allocated at least one medical personnel, and the service population is increased by one for every 1,000 people added. Staffing is carried out through open recruitment, re-employment of retired medical personnel, dispatched by superiors, targeted training, etc.
At present, many districts in Beijing have issued a notice of recruitment of rural doctors, among which the recruitment notice of Jinxing Health Center in Xihongmen Town, Daxing District, shows that under 68 years old with a college degree and holding a Beijing rural doctor practice certificate can apply. For rural doctors with outstanding abilities, Miyun District has relaxed the academic qualifications to secondary schools.
Equal pay for equal work, to the establishment, five insurances and one gold
In terms of treatment, the Notice requires that, in order to maintain the public welfare of village-level medical and health institutions, actively promote policy coordination between municipal and district governments in terms of village-level health personnel supplementation, salary and treatment, social security and appropriate talent training.
In accordance with the principle of "town management and village use", the town-level health personnel establishment is scientifically approved according to the number of administrative villages and the service radius, and the problem of insufficient personnel is solved through regional establishment and coordination or non-staff employment, contract management, financial guarantee treatment, government procurement of services, and the promotion of medical personnel in second- and third-level medical institutions to the middle and senior professional titles before the grass-roots level.
The "Circular" emphasizes that all districts should create conditions to ensure the treatment of "town management and village use" personnel, and implement "equal pay for equal work" and "five insurances and one gold". Districts that do not have the conditions for the time being can adopt the method of government procurement of services to ensure treatment, and dynamically improve the standard of treatment.
Previously, in the notice issued by Beijing Municipality to accelerate the replenishment of rural doctors, it was clear that the long-term idle business establishment would be given priority to the township community health service center, supplement the shortage of personnel in the township health institutions, and then dispatch the village clinic to provide services. And the implementation of "rural doctor post management", that is, fixed posts and no one.
That is to say, the establishment and treatment no longer follow people, breaking the so-called "iron rice bowl", and changing from personnel management to post management is of far-reaching significance, which also means that post competition will be more intense.
Different models, different treatment
As non-profit medical institutions, village-level medical and health institutions fulfill the public health, basic medical care and epidemic prevention and control responsibilities entrusted by the government.
The "Circular" emphasizes that the treatment of medical personnel should be properly and reasonably guaranteed. For village-level medical and health institutions and personnel with different management models, they should refer to the corresponding guarantee standards to implement treatment, and issue them in full and in a timely manner on the basis of assessment.
For the guarantee standards of "different models" (that is, the above-mentioned open recruitment and other methods), beijing municipality mentioned in the document on accelerating the supplementary of rural doctor positions that it can appropriately break through the existing rural doctor post treatment. Each district can combine the actual situation, and for rural doctor positions that are difficult to attract personnel such as remote mountain villages or difficult conditions, they can appropriately break through the existing government procurement service level (3500-5500 yuan / month) to ensure that the personnel are replenished in place.
It is reported that at present, Pinggu District of Beijing has implemented this policy, will recruit 139 village doctors for the community, according to the ordinary area, semi-mountainous areas, mountainous areas, providing generous subsidies ranging from 4000-5000 yuan (previously reported→ village doctor subsidies of 5000 yuan / month, pay five insurances, the local health commission "big money").
The satisfaction of villagers is linked to the treatment of village doctors
The above favorable policies all depend on whether the village-level medical institutions implement various services, and basic public health services have always been one of their key tasks, and the Notice also clarifies this.
The document mentions that village-level health personnel can work with the medical service team of the township health center to provide villagers with basic medical services such as health management, chronic disease prevention and treatment, medication guidance, rehabilitation nursing, etc., and can also sign contracts with villagers separately to carry out public health and basic medical services.
In addition, the township health center will supervise and guide the service content of village-level medical institutions throughout the process, mainly focusing on the following three aspects:
1. Strictly control the quality of basic medical and public health services, and regularly carry out service supervision and business guidance for village-level medical and health institutions and staff, including service attitudes, medical quality, and rational use of drugs.
2. Strengthen the assessment and evaluation of village-level medical and health institutions, formulate practical assessment and evaluation standards and strictly implement them. Assessment and evaluation can be divided into monthly, quarterly, semi-annual, annual and other different stages. At the same time, it is necessary to listen to the opinions of villagers extensively and take villagers' satisfaction as an important indicator for evaluating village-level medical and health services. The evaluation results are linked to the treatment of medical and health personnel at the village level.
3. Undertake the responsibility of medical insurance management, eliminate the running fees and leakage of medical insurance funds caused by inadequate management, and ensure the safety of medical insurance funds.
Overall, although this document is freshly released, many areas of Beijing have implemented the spirit of the document in advance, on the one hand, they are constantly building and renovating village-level medical institutions to provide a better working environment for village doctors; on the other hand, by improving the treatment of village doctors and providing social security for village doctors, they have increased the attractiveness of rural doctors' posts, which has played a good exemplary role in the whole country and is worth exploring and learning from other places.
