The team members perform their duties, some measure blood pressure and blood sugar, some diagnose the body, and the examinations are in an orderly manner, and they do health examinations for key groups in the jurisdiction free of charge.
Shen Shuyun, a contracted service recipient of family doctors, has benefited a lot from continuous health management services, and she often goes to the community health service center to participate in free clinic activities, "I participate in the annual physical examination on time, and if I usually have medication or any discomfort, I can contact my family doctor with a phone call, and he will solve everything." Shen Shuyun said.
In order to implement the contract performance service of family doctors, the Longzihu District Health Commission has improved a series of convenient measures, such as the mobile CT car service of Yan'an Street Community Health Service Center, and the bone density monitoring and gastrointestinal endoscopy of Caoshan Community Health Service Center, all of which have been carried out as special service projects and have been well received by residents.
In order to build a new order of hierarchical diagnosis and treatment of "primary diagnosis, two-way referral, acute and slow treatment, and upper and lower linkage", Longzihu District has grasped the needs of the community and formulated the "Personalized Service Menu", which can provide personalized or à la carte contract services according to the needs of residents, and the contract service has been transformed from a fixed time to a flexible cycle, providing residents with fair, accessible, systematic and continuous services.
At present, there are 178 "family doctors" in Longzihu District, covering 1 township health center, 7 community health service centers, 31 health service stations, and 17 village clinics, serving a total population of 260,000 people, of which 36,896 residents have signed contracts for paid services. Family doctors can solve the treatment of common, frequent and rehabilitated diseases for individuals and their families, and can provide door-to-door services for some key groups and patients with reduced mobility, which promotes the transformation of the medical and health service model to "health-centered". The health literacy level of residents has been continuously improved, and the contracted service of family doctors has built a "heart-to-heart bridge" between doctors and patients.
