Multiple medical records in one CT scan? The doctor reported the hospital in his real name for suspected insurance fraud

Loading...

Recently, the radiographer of Wuxi Hongqiao Hospital in Jiangsu Province reported to the Voice of China News Hotline 400-800-0088 that since last year, there have been some patient medical records in the hospital system who have not done imaging examinations in their departments at all.

All these medical records were completed by Dr. Chen from the hospital's physical examination center, and this doctor did not have the authority to operate the CT and MRI machines of the radiology imaging department. Doctors in the radiology department suspected that the hospital was suspected of falsifying medical records and defrauding medical insurance. In this regard, the reporter of the Voice of China of the main station conducted an on-the-spot investigation↓↓↓

There is a diagnosis in the medical record but there is no image and image number, and after being reported, there is an image and an image number

Wuxi Hongqiao Hospital in Jiangsu Province is a second-class general hospital and a designated unit of medical insurance.

Dr. Zhu Chenggang of the Radiology Department of the hospital reported that starting from July 2023, when he checked his past medical records, he found that some patients did not have imaging numbers in their medical records, nor did they have imaging images from computed tomography (CT) or MRI, but there were relevant content based on imaging diagnosis in the medical records.

△ The medical records of the problem taken by Dr. Zhu in March 2024 have relevant content based on imaging diagnosis, but there are no images and image numbers.

Zhu Chenggang said that these medical records were written by a doctor Chen from the hospital's physical examination center, and this doctor Chen is not usually involved in the work of the radiology department, and the imaging reports he issued do not have an image number, no images, and even do not mark the examination site.

Zhu Chenggang made an analogy, this is like a newborn born who has no name and no birth medical certificate but can get a household registration. "From a radiology point of view, such a result is simply not operable, because the CT machine cannot start the scan without entering the image number."

In the CT machine operation room, Zhu Chenggang demonstrated that in the operation interface, the patient's image number is required, if it is blank, the scanning button is gray and cannot be clicked, and the key becomes green only after entering the number, the operation can be continued.

The radiology department suspected that someone had bypassed them and directly modified the medical records from the background of the hospital system, and Dr. Li (pseudonym), an insider in the radiology department, told reporters that if the image number was not lost during the examination, the machine would not run the next step, and he could not check it. "There are only false reports, and there are no image numbers."

Zhu Chenggang showed reporters photos of the medical records of 99 "patients", the diagnosis time is concentrated from July to December 2022 and April 2023, and the age group is between 60 and 80 years old.

In March this year, Zhu Chenggang took a photo of the medical record without an image number and reported it to the hospital's higher-level management department in real name. While waiting for the results of the investigation, Zhu Chenggang found that the medical records he reported had image numbers and films.

△ After being reported, the false image and false image number are supplemented.

After more problems are discovered, the medical records of the problem can no longer be viewed with permissions

In June, Zhu Chenggang took photos of these medical records one by one. He told reporters that he carefully checked these image numbers, and the numbers only existed in the computer system, and these patients were not found in the handwritten record book of the radiology department, and the coding method of the image number in the computer system was also different from the daily coding method of the radiology department. In addition, he found other problems:

1

Many patients have images that are highly similar to others

In the reported medical records, in the medical records of Yang Moumou (72 years old) and Qiu Moumou (62 years old), there are diagnoses made based on the conditions of the films, but the corresponding images cannot be found. In June, their CT images were found again, and according to the system records, both of them had chest CTs done in April 2023, and it can be clearly seen on the film that the shape, size and position of the pendants of the two patients are basically the same.

Not only are the CT images of these two elderly patients similar, but Zhu Chenggang found that the 25-year-old woman Ye Mou, who had a CT scan in their department in 2021, also has a pendant that looks very similar, and with the naked eye, the three of them are almost identical.

△ 72-year-old Yang, 62-year-old Qiu and 25-year-old Ye, the films of the three are almost identical.

There are many films like this that seem to have a high degree of similarity. The system displays:

Patient Li Moumou took a chest CT on April 28, 2023, and her images looked highly similar to the examination images of the actual patient Liu Moumou in February 2024. The 74-year-old patient Mo took a CT scan of the chest and cervical spine on April 27, 2023, and her film looks highly similar to the film taken by the actual patient 37-year-old Ma Moumou in November 2023, and the necklaces can be seen in the film.

2

2 MRIs within 1 minute

In addition, through the hospital system query, it can also be seen that some patients have undergone an MRI examination within 1 minute after the normal MRI examination. Dr Lee said it takes at least 10 to 15 minutes for their machine to do an MRI of the lumbar spine.

△ Recording of 2 MRI (magnetic resonance imaging) within 1 minute.

3

The image corresponds incorrectly to the left and right directions of the diagnosis

Dr. Li told reporters that there are still such errors in some medical records: when the film was taken, the patient was facing the doctor, and the left side of the film was actually the right side of the patient, but the diagnosis of some patients said "infarction of the basal ganglia area of the left brain", but her film was shown on the right side.

Just after Zhu Chenggang reported these new situations to the higher authorities, he and his colleagues found that the radiology department no longer had the authority to view the images before August 2023, and the reason given by the hospital was an error caused by the system upgrade.

Later, Zhu Chenggang took the reporter to the office of the radiology imaging department, and the reporter saw that the reported medical records in the system could no longer be opened.

From "there is a diagnosis without images and image numbers", to images appearing after reporting, but there are many patients with images that are highly similar to others, to images and medical records that cannot be viewed. Zhu Chenggang suspected that the medical records of the patients involved had been revised many times, which was a falsification of medical records and fraudulent medical insurance.

The hospital has not yet made any response to this, and the Wuxi Municipal Medical Insurance Bureau said that it has requested the public security department to assist in the investigation

On July 12, the Wuxi Medical Insurance Bureau of Jiangsu Province said that it attached great importance to Zhu Chenggang's report, and went to Wuxi Hongqiao Hospital twice for on-site verification, but because the clues provided involved a large number of people and the problems reflected were a long time ago, the clues provided could not be confirmed and verified in the early verification, and they also specially coordinated and communicated with the Wuxi Municipal Public Security Department, and submitted the relevant clues to the public security department to assist in the investigation. If there is clear feedback from the public security department after verification, it will be notified in a timely manner.

Dr. Li from the Department of Radiology said that he was looking forward to the follow-up investigation, and that every change to medical records should be recorded in the hospital's underlying image archiving and communication system.

The reporter randomly dialed the phone numbers of patients left in 20 medical records, and three-quarters of them were empty or unreachable, and most of them said that they were not themselves. Some patients who answered the phone said that they were hospitalized in the hospital for a physical examination at that time. For example, Ms. Xu, a patient, said that she went to Wuxi Hongqiao Hospital for a physical examination because of back pain, but as for why she only did chest CT, Ms. Xu said that after a long time, her memory was not very clear.

The Fund Supervision Department of the Wuxi Municipal Medical Insurance Bureau said that the matter is still under investigation, and it is inconvenient to disclose the specific situation.

The reporter contacted Wuxi Hongqiao Hospital, and the hospital said that there would be a reply from relevant personnel, but as of press time, no response has been received.

Why are so many films so similar? Did you change the medical records? About half a year has passed since the doctor's real-name report, when will there be a definite investigation result? The follow-up situation, the Voice of China will continue to pay attention!
