TCM pulse diagnosis can objectively and comprehensively reflect the changing profile of the human body, but it is necessary to establish an objective and logical diagnosis model. The concept of multi-layer network was transferred to the relationship between pulse and disease, 25 pairs of pulse element nodes were extracted by input to construct a spatio-temporal pulse network, and under the guidance of the knowledge system of traditional Chinese medicine, the nodes were connected to each other, and the etiological layer, physical personality layer, psychological state layer, dysfunction layer and structural disorder layer were extracted, and finally the understanding of the general condition, etiology and pathogenesis of patients and the evolution process of the disease was output, so as to guide the clinical syndrome differentiation and form a disease diagnosis model dominated by the pulse element network of systematic syndrome differentiation and pulse. According to the characteristics of the etiological layer, the physical personality layer, the psychological state layer, the dysfunctional layer and the structural disorder layer, the spatiotemporal pulse network of the disease can be constructed, and the overall understanding of the patient can be obtained comprehensively, including: personality, constitution, psychology, main pathogenic factors and pathogenesis, disease judgment, etc. Therefore, through the logical thinking mode of multi-layer network, a comprehensive diagnosis "decision tree" covering multiple levels can be summarized and guided to form a multi-level treatment
Diagnosing diseases with abstract pulse diagnosis is difficult. Systematic syndrome differentiation and pulse science transfers the concept of multi-layer network to the relationship between pulse and disease, constructs the diagnosis and treatment mode of disease with pulse element network as the leading factor, and analyzes the characteristics of the etiological layer, physical personality layer, psychological state layer, dysfunction layer and structural disorder layer to clarify the key etiological pathogenesis and predict the outcome of the disease, in order to improve the diagnosis and treatment effect.
Traditional Chinese medicine (TCM) is the complex science that studies the human body's open complex giant systems. We classify the information, energy, structure and function abnormalities of the complex system of the human body as a certain type or a certain type of disease, so the diagnosis and treatment of diseases is also a systematic project, and each level is a complex network. The basis obtained from the pulse diagnosis of TCM is more comprehensive, which can clearly diagnose the disease and objectively construct the constant change profile of the whole human body. However, in the previous pulse diagnosis mode, the richness and accuracy of the diagnostic basis for pulse extraction depended largely on the doctor's theoretical level and experience, and the criteria for establishing the connection between pulse and syndrome type were often "imaged". We expect an objective and logical diagnostic model based on pulse elements and syndrome types. Multi-layer networks are one of the important directions in the research of complex networks. At present, the concept of multi-layer network is transferred to the relationship between pulse and disease, that is, the diagnosis and treatment mode of disease is constructed based on the pulse element network. The essence of this diagnostic model is the network logical relationship between the pulse elements and the diagnostic level, which can guide the diagnosis of TCM syndrome differentiation and Western medicine, formulate treatment principles and guide treatment, including the diagnosis network and disease network of systematic syndrome differentiation and pulse science.
1 The concept and characteristics of multi-layer networks
A multi-layer network is a network that contains multiple relationships. The multilayer network analyzes the spatiotemporal nonlinear correlation of complex systems through the study of various types of nodes and their edge relationships, including intra-layer edges and inter-layer edges. As a kind of topological structure, the importance of a layer of network nodes (node centrality) depends on the number and quality of neighbors pointing to the node, and is affected by the importance of layers and the connection mode between layers. The relative importance of each layer of the network in a multi-layer network is called layer centrality, which is a measure of the influence of points and layers. Ranking centrality quantifies the importance of influential nodes or layers in complex networks, which can be used to describe the structure and function of complex network systems.
2 Diagnostic network of systematic syndrome differentiation and pulse science
Under the guidance of system theory, systematic syndrome differentiation pulse science is a new pulse science system formed by using the basic principles of traditional Chinese medicine, cognitive psychology, modern informatics, physics, etc. It is a traditional Chinese medicine diagnostic technology that studies the mapping relationship between blood vessels, blood behaviors, phenomena and the physical and mental state of the body, the etiology and pathogenesis of traditional Chinese medicine and the disease parameters of Western medicine. Systematic syndrome differentiation and pulse science believes that the pulse is a complex system, and the spatial entity of the inch-mouth pulse is fractalized for positioning and diagnosis, and the pulse information felt under the finger is divided into 25 pairs of pulse elements with a single attribute for qualitative diagnosis. The 25 pairs of pulse elements are floating and sinking, up and down, left and right, inside and outside, late, knotting, straight, thick, long, high and deep, idling, scattering, moving, coming and going, slow, advancing and retreating, strong and weak, convex and concave, cold and hot, thin, slippery, withering, turbid, thick and thin, rigid and soft. The positioning diagnosis is five parts, six areas, seven layers and five belts. The five parts refer to the traditional three parts of the ruler plus the upper and lower parts of the inch: the main head of the inch, the main coke of the inch, the main coke of the main off, the main coke of the ruler, and the main waist and knee of the ruler. A complete pulse wave is divided into 5 zones horizontally, 6 zones vertically, and 7 layers radially, which closely correspond to various tissues and organs of the body, and form accurate positioning and diagnosis through the combination of pulse elements.
2.1 Decentralization of pulse elements
Decentralization does not mean that there is no center, but rather that in a system with many nodes, each of which is highly autonomous. Nodes can be freely connected to each other to form new connection units. Any node may become the center of the phase, but it does not have the mandatory central control function. The influence between nodes will form a nonlinear causal relationship through the network. The phenomenon or structure of the system is open, flat, and equal. The 25 pairs of pulse elements in systematic dialectical pulse science have the characteristics of decentralization. In the process of opening different sensory channels to extract pulse elements in turn, each pair of pulse elements should be extracted and "traversed", and the status of pulse elements is equal, and there is no dominant pulse element. See Figure 1.
Fig.1 Schematic diagram of the "decentralized" distribution of pulse elements
2.2 Centralization of diagnosis and syndrome differentiation - extraction of abnormal information
Although the pulse element is characterized by decentralization, the pathogenesis of the disease is centralized, and the pathogenesis of the main contradiction is the central pathogenesis, which plays a key role in the prognosis of the disease. After traversing 25 pairs of pulse elements, we can extract pulse elements with disease information and form a pulse element network to guide follow-up diagnosis and treatment. All pulse-shaped elements do not appear at the same time in a single disease process in the same individual, for example, in diseases with fever as the main manifestation, pulse-sign elements may be hot but not cold. The centrality of pulse elements in different networks is different, and the comprehensive analysis of these pulse elements can reflect the centrality of different etiologies and pathogenesis in the disease network, so as to clarify the key pathogenesis of the current state of the disease for syndrome differentiation and treatment.
2.3 The five-dimensional classification principle of spatio-temporal physics of pulse elements
The 25 pairs of pulse elements in systematic syndrome differentiation are classified according to the five dimensions of spatio-temporal physics: the spatial position of the pulse (bit), the number law (number), the spatial morphology (shape), the pulse wave and the vascular wall (movement), and the blood flow texture (quality). The spatial positions are: up and down, up and down, left and right, inside and outside; The elements of the law of numbers are: late number, knot; The spatial morphological elements are: straightness, thickness; Fluctuation elements: length, height, idleness, scattering, movement, coming and going, slow, advancing and retreating, strength and weakness, convex and concave; The texture elements are: cold and hot, thin, slippery, withered, turbid, thick and thin, rigid and soft.
2.4 Training and establishment of systematic syndrome differentiation and pulse diagnosis network
"Jin Kui Yao slightly · chest paralysis heart pain and shortness of breath disease pulse treatment nine" cloud: "The husband's pulse should be too late. Deviation from the "neutral state" of the body will produce pathological changes, and the pulse pattern will also change to the poles, which is represented by 25 pairs of pulse elements. Its direct representational meaning is stable and unchanged from individual to individual. The pulse is a non-equilibrium system, and there is no standard value for the constant and variable elements of the pulse image, and the pulse is often changed within a certain range, which is compatible with the physiological characteristics of the heavens, the earth and the person's own personality and physique. Beyond the "equilibrium state" and losing the "variable stress" is the change. Therefore, the establishment of the "feel" of the pulse element requires repeated and extensive training, the establishment of the "fuzzy swing" feeling and perception in the continuous comparison and summary, and the establishment of the individual's constant standard under the premise of overall commonality.
In the process of pulse diagnosis, according to the order of first always pressing and then pressing alone, the left hand and the right hand were examined in turn, and the two hands were compared at the same time. According to the five departments, six districts, seven layers and five belts, the concept of pulse diagnosis space is established, and the specific pulse element information in five dimensions is filled in with space, number, shape, movement and quality based on space and time series, so as to realize the qualitative on the basis of positioning and form a diagnosis network level.
3 Systematic syndrome differentiation and pulse science on the understanding of disease networks
Using the knowledge system of traditional Chinese medicine that has been mastered, the pulse elements are used as nodes, and the connections between specific pulse elements with a high degree of relevance are used as connecting edges, forming a multi-layer network of pulse elements, which is the mapping relationship between pulse elements and the state of the body. Different network layers represent different etiological and pathopathogenic meanings (pattern network layers), and different pattern network layers are organically combined through interlayer connections to form a highly generalized "pulse system" (i.e., disease network). The causal, evolutionary, juxtaposition, and timing relationships between the pattern network layers constitute the whole process of disease development. Clarifying the interlayer relationship is helpful to analyze, trace and restore the whole process of disease development, so as to have a clear understanding of all links, causes and results of disease development, and then establish the main pathogenic factors and mechanisms of disease.
Systematic syndrome differentiation and pulse science extracts pulse elements, and uses pulse elements to construct and analyze the human disease network, and finally obtains the corresponding pathogenesis, syndromes and overall understanding of diseases, and gives corresponding treatment prescriptions.
3.1 Organizational structure of the disease network
The organizational structure of the disease network refers to the logical thinking process of incorporating element nodes and establishing connecting edges, that is, extracting "evidence".
3.1.1 Etiological layer
The cause can be divided into external and internal injury. The pulse characteristics of the six external senses are as follows: the wind evil is "soft and slow", the cold evil is "rigid and cold", the wet evil is "thick and slippery and short", the hot evil is "hot and strong", the dry evil is "lack of glory and lubrication", the yin and summer are "just cold and thick and slippery", and the yang and summer are "floating and hot". For the internal injuries of the seven emotions, we can see the "movement" corresponding to joy, anger, worry, thought, sadness, fear, and shock, that is, the characteristic resonant waves. Diet stagnant "thick and hot and strong". Overwork is mainly characterized by "slackness".
3.1.2 Physical personality layer
Constitution and personality represent the basic physiological state of the human body, which determines the susceptibility and tendency of the disease and the outcome of the disease. The "Systematic Dialectical Pulse Theory" summarizes the characteristics of the vein elements of the "five-shaped person": the wood-shaped person is "straight and strong", the fire-shaped person is "hot and strong", the earth-shaped person is "thick and soft", the gold-shaped person is "thin, thin, thin and weak", and the water-shaped person is "thick and thick". The pulse elements of the personality of the "five states of people" are summarized as follows: people with the sun "float and move up", people with less yang "up, thin and long", people with too yin "sink and slow", people with less yin are "straight and thin and rigid", and people with yin and yang are "thick and gentle".
3.1.3 Psychological state layer
Different physique and personality are affected by different circumstances of individuals, and there are interlayer connections between the physique-personality layer and the psychological state layer. The state of psychological disorder is an important link in the process of constituting the disease. The systematic dialectical pulse system summarizes the pulse characteristics of five psychological disorder states: the pulse characteristics of mental malaise state are "slow to come and go, sluggish, deep, and short"; The pulse characteristics of irritability and anxiety state are "restless, numerous, high, short"; The pulse characteristics of depressed and uncomfortable state are "depressed, heavy, astringent, and short"; The pulse characteristics of the state of palpitation and restlessness are "alarm, counting, coming and going, driving"; The pulse of the state of excessive thinking is characterized by "thinking, constricting, straightening, and inward-curving". Psychological disorders can be superimposed on each other, cause and effect each other, and are more complex in the nodes and connecting edges of pulse elements. Generally speaking, the recent state of psychological disorder mostly appears in the left inch, and can be generalized to the ulnar if it is not relieved for a long time, and the early state of psychological disorder mostly leaves traces in the right ulna.
3.1.4 Dysfunctional layer
The dysfunctional layer can be further divided into the yin and yang imbalance layer, the qi imbalance layer, and the viscera dysfunction layer. The "Systematic Syndrome Differentiation Pulse Theory" summarizes the pulse elements of yin and yang imbalance as follows: "hot" in those with strong yang, "cold" in those with strong yin, "cold and weak" in those with yang deficiency, "withered" in yin deficiency, "cold, weak, scattered and slow" with "fine and withered" in those with yang damage and yin, "fine, astringent, dry and numbered" with "cold and sluggish" in yin and yang, and "thin, astringent" and "cold, short and weak" in those with yin and yang deficiency. The qi imbalance layer includes qi deficiency and qi imbalance, and qi deficiency is seen as "weak"; The pulse elements of qi imbalance are summarized as follows: qi stagnation syndrome "sinking, moving, and astringent", qi reversal syndrome "upper and coarse", qi trapping syndrome "lower and thicker", qi closure syndrome "deep and strong", and qi detachment syndrome "weak and scattered". Yang is "hot", yin is "cold", yang deficiency is "cold and weak", yin deficiency is "dry", and yin and yang deficiency are "thin, astringent, cold, short, and weak". The dysfunction of the viscera should be diagnosed in combination with the positioning of the five parts, six zones, seven layers and five bands of the pulse. If the heart yang is insufficient, it is manifested as "thin, weak, heavy, and cold" in the upper third of the left inch, and "sluggishness" at the beginning of the pulse wave.
3.1.5 Structural disorder layer
The structural disorder layer includes changes in tangible substances such as sperm, blood, jin and fluid, blockade of tangible evil such as phlegm, dampness and stasis, and solid lesions of visceral tissues. This layer includes the pathogenesis layer of the rise and fall of evil and the decay of traditional Chinese medicine theory, and also contains the understanding of diseases from the perspective of modern medicine. For example, the "thick and slippery" ones represent the phlegm turbidity connotation, the "sparse and slippery" ones represent the sputum saliva saliva, the "thin, slippery and radial artery tissues are like mud" to represent the flooding of water and dampness, and the "astringent" ones represent the internal resistance of blood stasis; Those who are deficient in jin fluid are "thin and dry", those who are deficient in blood are "thin and fine", and those who are deficient in essence are "thin and weak". To understand diseases from the perspective of modern medicine, it is necessary to use the positioning of five parts, six zones, seven layers and five bands, combined with pulse elements such as convex and concave, to identify whether the corresponding organs have tangible occupancy and the nature of the occupancy.
The above only summarizes the characteristic pulse elements of each layer, and complex combinations of pulse elements can be transformed according to the complex etiology and pathogenesis. On the basis of physique and personality, the state of psychological disorder acts on the human body to produce changes in qi, and the imbalance of qi leads to dysfunction and produces real evil, which causes structural disorder, and structural disorder can affect function, psychological state, and human physique and personality. Therefore, through the comprehensive analysis of pulse elements, the characteristics of the etiological layer, the physical personality layer, the psychological state layer, the dysfunctional layer and the structural disorder layer can be extracted, and a multi-layer network for disease understanding can be formed.
3.2 Temporal nature of disease networks
The disease is in the process of dynamic evolution, and the evolution of the etiology and pathogenesis constitutes the characteristics of the disease seen at the current time node, and it is very important to clarify the temporal sequence of the etiology and pathogenesis to guide treatment. Knowing its "coming" can grasp the evolution law of etiology and pathogenesis, and treat its underlying etiology and pathogenesis, which often achieves twice the result with half the effort; Knowing its "go" can judge the future development and prognosis of the disease, which is very important to grasp the disease situation and truncate the transmission. The physique and personality layer is at the bottom level, which constitutes the physiological and pathological basis of the human body. The etiological layer can play a role in turning on the disease and maintaining the disease process. The etiology acts on the personality constitution, resulting in a state of psychological disorder, dysfunction of the function of qi, and eventually leads to structural changes. Gas change often precedes deformation, so the dysfunctional layer lies beneath the structural disorder layer. Each layer affects each other through interlayer connections, and when the qualitative change occurs when the limit is broken, a new network level can evolve on this basis. Therefore, the complex network of diseases is not a simple two-dimensional structure, but a four-dimensional structure of three-dimensional spatial structure superimposed on time series.
3.3 Formation of diagnostic levels
The relationship between pulse elements and disease network levels presents a "one-to-many" mapping pattern, that is, a pulse element may have representational significance in the etiological layer, physical personality layer, psychological state layer, dysfunction layer and structural disorder layer. The patient's pulse collected in clinical practice is a collection of multiple pulse elements, in which multiple pulse elements will map a certain aspect. Through the organic combination of pulse elements, different levels of common mapping are obtained, and multi-level organizations form a diagnostic level. For example, the pulse element of "rigidity" indicates an increase in the hardness of the blood vessel wall. Due to the external cold evil, the condensation stagnation, the dilation of the blood vessel wall can be reduced, combined with the pulse element "cold, convergence", at this time, it is generally more common in the left inch, and the etiological layer diagnosis of "external cold evil" can be made in the etiological level. If the pulse elements are "weak, scattered, floating, and cold", the diagnosis of dysfunctional layers such as overwork, lack of yang energy, and endogenous cold and evil can be made. If combined with the pulse elements of "astringent, fine, and astringent", it mostly indicates that the person's personality is cautious and overthinking, which is the diagnosis of the constitution personality layer. If you see the blood vessel wall on a certain part of the ruler alone, it can indicate that the qi stagnation and blood stasis and meridian stasis of the part are common, which is common in various limb meridian pain syndromes. The pulse elements "strong, strong, convergent, driving, and long" characterize hypertension. Through the existing knowledge system of TCM and the above-mentioned mapping relationship, different levels can be formed by combining the pulse elements to establish the connecting edges.
Therefore, through the logical thinking mode of multi-layer network, a comprehensive diagnosis "decision tree" covering multiple levels can be summarized, and a multi-level treatment can be guided to achieve the corresponding pulse prescription and pulse needle.
4 Case Count
The patient, a 72-year-old female, was first seen on 23 August 2022. Chief complaint: difficulty falling asleep for more than 40 years, aggravated for more than 10 years. Tick syndrome: difficulty falling asleep every day, easy to wake up from shallow dreams, low mood, upset and easy to cry, episodic tremor of the right hand, trembling of the right leg, cough and phlegm, suffocation, nak, loose stools, frequent urination. He had a history of bronchitis for more than 50 years, a history of hyperlipidemia for more than 10 years, and a pituitary tumor for half a year after surgery. The lips are dark, the tongue is dark red, and the moss is yellow and dry. Overall pulse: upper, sick, fine, astringent, thick, astringent; Local pulse: the right inch is hot, agitated, turbid, dilated second-stage bump; The right pass is slippery and weak; The right ruler is a cold, heavy, thin, and angry resonant wave; The left inch is inward, less advance and more retreat, and slack; The left guan is convex, hot, and depressed; The left ruler is dry, cold, heavy, and rigid. Western medicine diagnosis: insomnia. Traditional Chinese medicine diagnosis: insomnia, hyperactivity of liver yang. Treat the liver and latent yang, soothe the liver and strengthen the spleen.
Acupuncture is used to relieve the knots in the heart, and the patient immediately feels that the chest tightness is reduced, the overall relaxation is reduced, and the resonance wave is reduced. For the internal administration of traditional Chinese medicine, Gastrodia gastrodia hook vine drink is added or subtracted: Gastrodia gastrodia 12g, Hook vine 15g, Cassia stone 15g, Eucommia ulmoides 15g, Oxknee 12g, Mulberry parasitism 15g, Skullcap 12g, Motherwort 15g, Cocos 12g, Nightshade 12g, Bupleurum 12g, Xiangfu 15g, Atractylodes 20g, Citrus aurantium 12g, Yam 20g, Cinnamon 12g. 14 doses, 1 dose per day, decoction in water.
Follow-up: After taking the medicine, the patient felt calm, fell asleep slightly improved, cough and suffocation decreased, sagittal qi increased, and loose stool with sticky foam. Pulse: Up, disease, and resonant waves tend to decrease slightly. Above: minus cinnamon branches, add 6g of cinnamon, 12g of angelica, and 12g of Chuanxiong.
Press: Use multi-layer network for logical analysis. Physical personality layer: the overall pulse is thin and constricted, and the left inch is curved inward, indicating that the patient is usually more cautious, combined with the resonant waves of depression and anger, indicating that the patient is wronged and angry but does not tolerate it, which is biased towards the personality of a gold-type person. The overall pulse is upper, sick, inch, and cold at the end, indicating that the patient's constitution is hot at the top and cold at the bottom, and the left ulnar is dry when the kidney water is insufficient, and the right ulnar cold is seen when the kidney yang is insufficient. Etiological layer: The resonant wave of anger is generalized to the right ulnar, which shows that the patient has been angry for a long time. Psychological state layer: restlessness and depression, the patient is angry and depressed for a long time and turns into fire, both depressed and uncomfortable, and irritable, showing the patient's ambivalence of wanting to vomit quickly and unable to express his chest directly. Dysfunctional layer: the overall pulse is up, the disease is the upward reversal of the liver yang carrying qi, and at the same time, the left inch advances less and retreats more, which indicates that the heart qi is insufficient, and the blood supply to the heart and brain is poor, which is related to the patient's long-term insomnia. Structural disorder layer: the overall pulse is thin, astringent, and thick, locally hot and slippery, and the patient's blood lipids are high and there is phlegm turbidity; The overall pulse is astringent: qi stagnation and blood stasis; The second-stage bulge of the right inch dilation is a sign of chronic bronchitis, and the left ulnar gang corresponds to the discomfort of the lower limbs.
Although the patient came to the clinic with insomnia, the logical analysis of the multi-layer network of systematic syndrome differentiation pulse showed that insomnia was only a partial representation of the disease network. First of all, because the golden type person is more forbearing, he has encountered a bad situation, and he has been depressed for a long time, resulting in a series of pathogenesis and stagnation of qi and qi. Liver depression is caused by the spleen to produce phlegm turbidity, liver depression turns into fire to overcome the lungs, and lung qi is unfavorable and coughs, so chronic bronchitis does not heal for a long time. Depression and heat burn the fluid for a long time, the muscles and veins are not nourished, the kidney yin is lost, the liver yang can not be contained and the wind moves internally, and the limbs tremor. Yin damage and yang damage and the patient's old age and physical weakness, kidney yang deficiency and insufficient warmth, so loose stools and excessive urination. The initial insomnia of the patient may be due to sleep disorders caused by poor mood, but in the long run, the liver and yang qi mechanism is overcharged, and the yin and yang are out of balance, resulting in chronic insomnia. As a result, a therapeutic level has been formed: acupuncture Neiguan acupoint and Hegu acupoint to relieve heart knots, regulate qi and relieve depression; The treatment of gastrodia hook vine to calm the liver and latent yang, first, to make the impatient part of the patient's personality calm; Second, the pathogenesis of liver depression is at a relatively low level, which is the key node of treatment. The third is to dive the liver yang, clear away heat and calm the nerves, restore the combination of yin and yang, and improve sleep; Fourth, it calms the liver and calms the wind, calms the tremor, and adds cinnamon branches to warm the tendons and veins; The fifth is to add atractylodes and yam to dry and damp and strengthen the spleen. The second diagnosis reduces cinnamon branches and cinnamon to warm the kidney yang, leads the fire to the yuan, and adds angelica and Chuanxiong to dissolve blood stasis and blood. The whole approach is not only to address the local problem of insomnia, but also to start from the underlying pathogenesis of the patient, and there are corresponding treatment prescriptions at each disease level, reflecting the holistic and systematic view of traditional Chinese medicine. In the whole, the key point of the pathogenesis: hyperactivity of liver yang progressed from liver stagnation, and this was taken as the focus of treatment. From the pulse of the left inch, it can be seen that the patient's cardiovascular disease has appeared and may progress to cardiac insufficiency in the future, which reflects the grasp of the future process of the disease by systematic syndrome differentiation and pulse differentiation.
5 Concluding remarks
In the process of analysis, the characteristics of the etiological layer, the physical personality layer, the psychological state layer, the dysfunctional layer and the structural disorder layer are established, and the spatiotemporal pulse network is constructed, so as to obtain the general situation of the patient and the development process of the disease, and form a diagnostic decision tree. Under the guidance of the systematic syndrome differentiation and pulse diagnosis model, the key etiology and pathogenesis can be identified during treatment, and the prognosis of the disease can be predicted from the perspective of constitution and personality, which can improve the diagnosis and treatment effect.
