It is reported that the Health Development Research Center of the National Health Commission reviewed 28 typical cases from more than 100 cases across the country and collected them into the register for reference and reference of personnel engaged in the management of chronic diseases at the grassroots level.
In recent years, the District Health Bureau has vigorously promoted the integration of medical prevention, medical care and elderly care, integrated high-quality medical and public health resources, promoted medical experts from tertiary public hospitals and public health backbones of the CDC to sink to the grassroots level, and took the management of chronic diseases such as hypertension and diabetes as a breakthrough to optimize the health management team, improve the service process, and innovate the management mode of chronic diseases at the grassroots level with the help of information support, carry out the integration of medical and prevention projects for chronic diseases, and strengthen the diagnosis and treatment and health file management of primary medical institutions, health education, health examination, chronic disease follow-up, Public health services such as family doctor contracting, multi-channel and all-round in-depth integration of medical treatment and prevention, and continuous improvement of the ability to manage chronic diseases at the grassroots level.
By the end of 2023, the health literacy level of residents in Wujin District was 40.8%, the electronic health record filing rate was 96.3%, the utilization rate was 66.5%, the urban and rural community standardized health management service rate for the elderly aged 65 and above was 78.9%, the grassroots standardized management service rate for hypertension patients was 85.5%, the blood pressure control rate of the management population was 76.7%, the grassroots standardized management service rate for patients with type 2 diabetes was 84.9%, and the blood glucose control rate of the management population was 60.8%. The key objectives ensure quality while completing quantitative tasks, and effectively promote the high-quality development of chronic disease patient management.
