A woman in her 50s complained of "aphasia with inflexibility of the right limb movement for 3.5 hours" in the Department of Neurology, and her family members complained that the patient was unconscious, unresponsive to shouting, nausea and vomiting several times, and had a history of diabetes and hypertension in the past, because the patient had a stroke after waking up, and the specific onset time could not be determined. The best treatment option is interventional therapy, and after obtaining the consent of the patient's family and improving the relevant preoperative examination, super-selective cerebral arterial transluminal thrombectomy is planned.
The interventional team of the Department of Neurology immediately started the endovascular treatment plan, the medical team of the catheterization laboratory and the hand anesthesiology department quickly took place, and immediately performed cerebral angiography, and the initial segment of the patient's left middle cerebral artery was occluded, and the stump was visible, and thrombectomy was performed immediately, and a strip-shaped red thrombus about 1cm long was seen after the thrombectomy stent was withdrawn, and the re-examination showed severe stenosis of the initial segment of the left middle cerebral artery, and balloon dilation + stent implantation of the stenosis segment was given, and the re-test of the contrast showed that the middle cerebral artery had smooth blood flow and good distal blood flow.
After more than 10 days of active treatment by medical staff from the Department of Neurology and Intensive Care Medicine and the close cooperation of his family, the patient finally passed the acute stage of cerebral edema, his consciousness gradually improved, he was able to communicate with his family members simply, and after follow-up rehabilitation treatment, the patient was now able to stand alone.
